Discovered in the 1960s, selegiline is a selective MAO-B inhibitor that slows the brain’s breakdown of dopamine, easing motor symptoms for some people with Parkinson’s disease. Today it is widely used, often as an add-on to levodopa or in specific depressive regimens, and it is available in oral and transdermal forms.
Selegiline blocks monoamine oxidase B, the enzyme that breaks down dopamine in the brain. This raises dopamine levels and can improve motor function in Parkinson’s disease.
At low doses it mainly inhibits MAO-B and spares MAO-A, which reduces the risk of dietary interactions; at higher doses or certain formulations MAO-A inhibition can occur, raising tyramine-related concerns. The available forms include oral tablets and a transdermal patch; the patch delivers steadier exposure with fewer peak effects.
In Parkinson’s disease, selegiline is used as an early monotherapy in some patients and as an adjunct to levodopa in others to enhance effect and extend the benefit of levodopa.
