Rifaximin is a rifamycin antibiotic that stays largely in the gut and is poorly absorbed into the bloodstream. It inhibits bacterial RNA synthesis and is primarily used to treat traveler's diarrhea caused by noninvasive Escherichia coli and to manage hepatic encephalopathy and irritable bowel syndrome with diarrhea.

What Rifaximin is

Rifaximin belongs to the rifamycin family. It acts locally in the gut by inhibiting bacterial RNA polymerase, with minimal systemic absorption. This localized action reduces systemic side effects while targeting gut bacteria. It is commonly formulated as tablets taken by mouth and is designed to minimize impact on non-gut bacteria.

What conditions it treats

Rifaximin is used to treat traveler's diarrhea caused by noninvasive E. coli strains. It is also approved for irritable bowel syndrome with diarrhea (IBS-D) and for preventing episodes of hepatic encephalopathy in people with liver disease. It is not a universal antibiotic and should be used for conditions that affect the gut or gut-related symptoms, as prescribed by a clinician. Follow the prescribed plan and do not expect it to treat unrelated infections.

Practical usage guide — timing, food, missed doses, storage

Take exactly as prescribed by your clinician. Rifaximin tablets are swallowed whole with a full glass of water; do not chew or crush. The timing of doses follows your prescription, but many regimens allow with or without meals. If you can take it with meals, that may help reduce stomach upset. Do not change the dose or timing yourself. If symptoms worsen or do not improve within a few days, contact your health professional. Finish the full course unless told otherwise. Store in the original packaging at room temperature, away from moisture and heat, and out of reach of children. Do not use rifaximin after the expiration date. Missed dose: if you miss a dose, take it as soon as you remember unless it is almost time for the next dose. Do not double up to catch up. If it is almost time for the next dose, skip the missed dose and resume your regular schedule.

What to watch for — side effects in plain language

Common side effects include nausea, gas, stomach upset, and headache. Some people may experience dizziness or a mild rash.

Call your doctor if you have severe diarrhea, persistent vomiting, signs of an allergic reaction (swelling of the face or lips, trouble breathing), or if you notice fever or severe abdominal pain. If symptoms persist after finishing treatment, contact a health professional for advice.

When to consult a doctor

Consult a healthcare professional before taking rifaximin if you are pregnant, breastfeeding, have liver or kidney disease, or take other medicines that affect the gut or interact with antibiotics.

Seek urgent care if you develop signs of a severe allergic reaction, a sudden severe rash, or if you have a new, persistent diarrhea that is bloody or signs of dehydration. Also consult if your symptoms do not improve after starting therapy or worsen after a few days.
